A flush trim router bit has the bearing at the end of the bit, allowing you to trim material flush to a template placed on top of the workpiece. A pattern bit has the bearing at the shank (top) of the bit, so the template is placed underneath the workpiece. Both are used for template routing, but the position of the bearing determines where the template goes.
