A straight bit is used in a router to make straight cuts, grooves, or dados in wood. A flush trim bit has a bearing that rides along the edge of a template or workpiece, allowing the cutting blades to trim material flush to that edge. The key difference is that flush trim bits are guided by a bearing, while straight bits are not.
